Required Documents

1. Applicants are required to submit the following documents directly to the ACFD with the application form:
• Applicant’s start date and the date of completion of the specialty program;
  
• Official transcripts from all dental and dental specialty programs;
  
• Original notarized copies of the degree/diploma/certificate from all dental and dental specialty programs;
  
• Original certified translations, or original notarized copies of all transcripts and diplomas/degrees/certificates into English or French;
  
• Completed self evaluation checklist in the applicant’s specialty;

2. Applicants are also required to contact World Education Service (WES) and apply to arrange for credential verification of the following documents:
   • Degrees/diplomas/certificates
     
   • Official Transcripts from the dental specialty program
