Application
Deadline: August 15, 2010
The application process
for the DSCKE consists of 2 components:
- Application
to Association of Canadian Faculties of Dentistry (ACFD):
The completed ACFD Application Form, the application fee
and ALL of the required documents outlined on page 3 &
4 of the ACFD Application Form must be received by the
ACFD office by the application deadline. The Application
Form must be completed legibly and must include your current
address, telephone number and email address in case there
is a need to contact you.
No late applications will be accepted and failure to supply
all of the documentation will result in rejection of the
application.
Upon receipt of your application you will provided with
an identification number. Please use this identification
number in all your correspondence with the ACFD.
- Application
to World Education Services (WES):
You must apply to WES (http://www.wes.org/ca/acfddsp.asp)
and pay the necessary fees to have the required documents
sent by your institution directly to WES for verification
All documentation must be received by WES by the deadline
date. No late applications will be accepted. Failure to
supply all of the documentation by the deadline date will
result in rejection of the application. WES will verify
the authenticity of the documents and forward a report
to ACFD

Examination:
November 13 - 14, 2010:
The registration
deadline for the examination is August 15, 2010. No late
registrations will be accepted.
To register for an examination, you must notify the ACFD
office in writing and pay the examination fee by the deadline
date. When requesting registration for the examination you
must indicate your choice of examination location and language.
No request for changes to location or language will be accepted
after the registration deadline.
Examination centres may be established at Canadian Faculties
of Dentistry provided a minimum of 10 candidates apply at
a location.

Withdrawal
from the Examination:
In circumstances
when a candidate must withdraw from the examination, the
ACFD office must be notified in writing as soon as possible.
If notification to withdraw is received by the ACFD
office in writing prior to the registration deadline the
full Examination fee will be refunded.
Withdraw from
the examination after the registration deadline, and up
to 15 days prior to the examination.
Withdraw from
an examination within 14 days, or fail to appear for the
examination.
A candidate may
request a refund of the examination fee, less the withdrawal
fee as set out by the ACFD, if they were unable to attend
the examination due to medical reasons. Candidates who notify
the ACFD of withdrawal in writing at least 24 hours prior
to the examination and submit an original medical certificate,
signed by a physician to the ACFD within thirty days of
the examination date qualify for a refund less the withdrawal
fee of $100.00. Candidates who fail to appear for an examination
or are required to leave the examination centre during the
examination due to medical reasons must notify the ACFD
in writing within seven days of the examination and must
submit an original medical certificate, signed by a physician,
to the ACFD within thirty days of the examination date to
be eligible for a refund of 50% of the examination fee.

Manual
Re-scoring:
Candidates who
wish to have their examination remarked can request a manual
re-score. Requests for a manual re-score must be submitted
to the ACFD office in writing within 3 months of the examination
date. |

All payments must be made
in Canadian funds to the order of the Association of Canadian
Faculties of Dentistry. Funds from outside of Canada must
be sent in the form of a money order or bank draft. The
ACFD does not accept credit card payments. |
