Required Documents

The following documents must be submitted with the completed application form and fee:

1. Applicant’s start date and the date of completion of the specialty program;
2. Official transcripts from all dental and dental specialty programs;
3. Original notarized copies of the degree/diploma/certificate from all dental and dental specialty programs;
4. Original certified translations, or original notarized copies of all transcripts and diplomas/degrees/certificates into English or French;
5. Completed self evaluation checklist in the applicant’s specialty;
6. Two photographs.
