Required Documents

The following documents must be submitted with the completed application form and fee:

1. Official transcripts from all dental and dental specialty programs;
2. Original notarized copies of the degree/diploma/certificate from all dental and dental specialty programs;
3. Original certified translations of all transcripts and diplomas/degrees/certificates into English or French if required;
4. Completed self evaluation checklist in the applicant’s specialty;
5. Two photographs.
