Confirmation of Graduation

Applicants must download and complete Section 1 of the Confirmation of Graduation form. This form must be sent to the university from which the applicant received their specialty diploma for completion of Section 2. Upon completion of Section 2, the university must send the completed form to the ACFD in a sealed envelope.
