Eligibility Examination Date - 2010

EXAMINATION DATE:                                    February 6, 2010

APPLICATION DEADLINE DATE:                December 1 , 2009

Application forms with all required documentation, fees and Request for Registration forms must be received by the ACFD office no later than December 1, 2009. Regardless of postmark or pickup date, any application, documentation, or Request for Registration form that fails to meet this requirement will not be processed.

It is the applicant’s responsibility to ensure that the application is completed properly and submitted with all of the required documentation by the deadline date.

Examination Locations

  • Eligibility Examination centres may be established at Canadian Faculties
    of Dentistry provided a minimum of 10 candidates apply at a location.
  • Eligibility Examination centres may be established outside of Canada provided
    that six weeks in advance of the application deadline, a minimum number of 50
    registered candidates request the same centre, and that an acceptable examination
    location with acceptable security can be established.

NOTE:

  1. Requests to change an examination centre or preferred examination language must be submitted in writing to the ACFD office PRIOR to the Application Deadline Date.

  2. Individuals who have previously registered for the Eligibility Examination do not need to submit a new application form, application fee, or required documentation. These individuals should forward a Request for Registration form with the examination fee to the ACFD office prior to the Application Deadline Date.

 

Eligibility examination fees - 2010

The Application process for the Eligibility Examination consists of two components:

APPLICATION

Individuals who have previously registered for the ACFD Eligibility Examination are not required to provide a new application form and do not have to pay the application fee. Please forward a Request for Registration with the examination fee.

APPLICATION TO WORLD EDUCATION SERVICE (WES)
Applicants are required to apply to WES for a Document by Document evaluation prior to the application deadline date.

Note: Individuals who have previously registered for the ACFD Eligibility Examination are not required to obtain the Document-by-Document evaluation.

RE-SCORING FEE

Candidates who wish to have their examination remarked can request a manual re-score. Requests for a manual re-score must be submitted to the ACFD office in writing within 3 months of the examination date.

ALL PAYMENTS MUST BE IN CANADIAN FUNDS TO THE ORDER OF THE ACFD.
FUNDS FROM OUTSIDE OF CANADA MUST BE SENT IN THE FORM OF A BANK DRAFT,
NOT A CHEQUE
. THE ACFD DOES NOT ACCEPT CREDIT CARD PAYMENTS.
